Admission Requirement
For New Student
- Application Form (4 pages)
- Two Photographs
- Payment of Registration fee - $300
- Previous School's Record
- Copy of Student's Passport First page and Visa page (if any)
- Copy of Parent's Passport First page and Visa page (if any)
- Copy of Student's Birth Certificate
- Copy of Parent's ID card (for Thai Citizen only)
- Copy of House registration (for Thai Citizen only)
- Copy of Medical Record ( Immunizations, Specific health concerns)
- Accident Insurance (required)
- QSI Uniform and PE Uniform (required)
- Locker Key (required for 12 years old and above)
- Lunch (optional)
- School Bus (optional)
Note, Following the submission of a completed application torm and payment of the registration fee, all students age 6 years old and oldder will be given reading, language arts and mathematics placement tests by an administrator or a classroom teacher. These tests do not determine eligibility for enroliment. Instead, the results of the tests will help the professionals at QSIP determine appropriate "starting points" for the learning program of the student.
